1) Family and
Community Partnerships/Home Visits
a) Assists
families to identify and cultivate strengths, and to identify and develop a
plan for addressing family challenges.
b) Works with families
on a one-to-one basis during 32 scheduled, 90-minute visits per operational
year.
c) Maintains
confidentiality of family records and information received at all times.
d) Orders,
maintains and tracks all supplies through Home-Based Coordinator.
e) Schedules
Home-Based Coordinator to accompany Home Visitors on at least one home
visit per operational year.
f) Schedules
resource staff to accompany Home Visitor on weekly visits when deemed
necessary by Home Visitor and/or Home-Based Coordinator.
g) Attends all
scheduled Head Start and Home-Based Program meetings.
h) Encourages
parents to attend appropriate Head Start meetings, and provides
transportation when necessary.
i)
Recruits families each spring for the next operational year, as
needed.
j) Assists
in maintaining a Home-Based Program waiting list of children who are
medically cleared to fill vacancies within thirty days.
k) Updates the
Family Partnership Agreement with returning families within 30 days of the
operational year, and renews the Family Goal Sheets.
l) Completes
the Family Partnership Agreement with each newly-enrolled family within 90
days of the beginning of the operational year, and develops the Family
Goals Sheet.
m) Completes the Community
Assessment forms with all enrolled families, as requested.
n) Secures
necessary resources to meet family social services and health needs,
ongoing.
o) Serves as
liaison and advocate for families with local community agencies and
organizations, as necessary.
p) Provides
families with an up-to-date information directory of community resources,
annually
q) Provides
referrals to community resources, and provides transportation when necessary,
to assist parents in gaining access to the referred services.
r) Provides
information and training for parents regarding child abuse and neglect.
s) Reports
instances of child abuse or neglect to local Child Protective Services
agency and to Home-Based Coordinator, as necessary.
t) Reports
crisis situations to Supervisor and works to develop a plan for assistance
for the family in crisis, as needed.
u) Organizes the
Home-Based Parent Committee annually
v) Advises the Home-Based
Parent Committee Chairperson in planning meetings, activities and training,
monthly.
w) Conducts election of
officers for Parent Committee, annually
x) Assists in
providing Parent Committee officer training, annually
y) Notifies parents
of Parent Committee meetings, monthly
z) Notifies
supervisor of elected officers and representatives to Policy Council and
Service Area committees.
aa) Develops Parent Training Plan based on tally of
Parent Interest Surveys and mandated training, annually.
bb) Completes
and tallies the Parent Interest Survey within the first 30 days of the
operational year.
cc) Organizes a calendar of parent
activity projects and meetings, monthly
dd) Submits parent committee agenda and minutes,
treasurer’s report and sign-in sheet to Supervisor after each meeting.
ee) Provides Home-Based Newsletter, monthly.
ff)
Submits Home-based family and Community Partnership Monthly Report
at last staff meeting of the month.
2) Child
Development
a) Prepares
developmental materials and loans them to parents, weekly
b) Provides
parents with training on topics related to child development and guidance.
c) Assists
parents in developing ways to use items found around the house as teaching
tools, weekly.
d) Provides
children with a group socialization experience of 2 ½ hours, twice a month.
e) Administers
screening and assessment instrument to all new and returning children
within 30 days of enrollment.
f) Repeats
screening instrument for all children who score below the cut-off score in
30 days.
g) Implements
plan to use screening tool results as a basis for follow-up activities to
be documented in the Home Visit Plan.
h) Completes the
Creative Curriculum Continuum three times per year, as scheduled
i)
Prepares the Creative Curriculum Planning and Progress Report with
the
parent’s
assistance three times per year as based on the Creative Curriculum
Developmental
Continuum.
3) Health
a) Submits
physical examinations and immunization records to Home Based Coordinator on
incoming children at the time of enrollment.
b) Completes
in-take interviews on new children, annually
c) Schedules dental
appointments for all children
d) Instructs
parents and children in basic dental hygiene skills, and distributes new
toothbrushes, as available
e) Obtains, and
keeps readily available, emergency telephone numbers for all children for
field trips and group sessions.
f) Reviews
and provides training in first aid and fire safety with parents and
children, annually.
g) Performs
weekly health inspection for each child.
h) Teaches and
models good hand washing techniques.
i)
Discusses nutritional values during food experiences at home or at
group sessions, taking into account and respecting cultural differences.
j) Uses
food experiences as a means of teaching language arts, colors, math,
science, social skills, and hygiene practices.
k) Uses
puppets/role play to introduce health services (dental care, immunizations,
etc.) as a way to calm children’s fears
l) Refers
children and families for nutritional counseling when weight or nutrition
is more of a concern than for a typical child.
m) Provides families, when
necessary, transportation to medical and dental services.
n) Secures
necessary resources to meet family health needs
o) Make
referrals to Disabilities or Behavioral Health Coordinator, as needed.
4) Supports a
team approach in serving as a member of the Education Staff
a) Attends
appropriate service area committee meetings when scheduled.
b) Attends
in-service training, conferences, workshops, etc. upon request.
c) Actively
participates in his/her own professional development long-range plans.
d) Meets with
supervisor bi-weekly or upon request.
e) Serves as a
member of the Home Based Child Development Team.
5) Record
Keeping
a) Records and
updates the Family Goal Sheets, three times during the program year.
b) Maintains and
updates family records throughout the year, as necessary.
c) Develops Home
Visit Plans based on identified family needs, with parent participation,
weekly.
d) Submits
completed health records to Home-Based Coordinator, on a bi-weekly basis
during the first 90 days of the operational year.
e) Submits
within 30 days of enrollment, completed health records on new children
enrolled after ninety days into the program year.
f) Provides
all required records to Home-Based Coordinator at bi-weekly staff meetings
as outlined in the yearly Home Based Timetable.
g) Maintains a
system for tracking results of family referrals to community services
agencies, ongoing.
6) ESSENTIAL KNOWLEDGES, SKILLS
AND ABILITIES
a) Must
possess knowledge of the social service and health resources available for
a specific
community as well as the wider Roanoke Valley
area.
b) Attends
community services meetings and networks with other agencies to facilitate
services to families.
c) Has a
working knowledge of the law pertaining to child abuse and neglect and can
present this information to families in an
effective manner.
d) Has good
knowledge of adult learning principles and can use this knowledge to
infuse the parent training programs with interesting activities that relate
to families
with pre-school children.
e)
Understands and maintains good interpersonal relationships with parents in
order to
organize a parent committee team and provide
meaningful home visits.
f.) Displays
a non-judgmental attitude toward children, adults and families, focusing
on family strengths and achievements.
7) Adheres to NAEYC Code of Ethical
Conduct as adopted by TAP Head Start.
