Coordinates services for children with suspected
or diagnosed disabilities and their families.
a)
Works with Family Services staff to recruit and enroll
children with disabilities.
b)
Meets with staff and parents prior to enrollment of
children with disabilities to discuss special needs the child
may have, and to assure staff that necessary materials,
training, and/or equipment will be provided as needed to ease
transition.
c)
Works with teaching staff to implement IEP/IFSP goals in
the classroom. Observes in the classroom and provides
suggestions to classroom teachers to ensure inclusion of child
in regular activities as much as possible.
d)
Works with Education Coordinator and center teams to
ensure implementation of Disabilities referral process.
e)
Works with Supervisor of Education & Transition and
Training Coordinator to develop plans for staff training related
to inclusion of children with disabilities in regular Early Head
Start classrooms or Home Base Socializations.
f)
Ensures that adaptive equipment is provided for children
as prescribed.
g)
Works with Transition and Training Coordinator to ensure
smooth transitions from EHS or HS for children with special
needs.
2. Ensures
that each child and family has access to a medical and dental
home
a) Provides
resource list and referrals to physicians and dentists in the
TAP service area to Family Development Specialists.
b) Monitors
records to ensure that eligible children have medical insurance.
c) Monitors
records for compliance with Virginia licensing and Head Start
Performance Standards concerning health and dental requirements.
3) Coordinates with Family Development Specialists to
provide health and dental services to enrolled children
a)
Coordinates follow-up care for medical, dental, and nutritional
needs of enrolled children and families.
b) Meets monthly
with Family Development Specialists to review children’s health
and dental needs.
c) Assists in
referral of children as needed with health and needs
d) Plans with
Supervisor of Health Services and Dental Health Coordinator for
dental exam, Fluoride treatment, and dental treatment as needed
e) Assists with
or provides transportation of children to and from follow-up
dental appointments as needed.
4)
Monitors health records to ensure compliance with performance
standards and state licensing requirement
a) Prints,
reads, and distributes monthly reposts concerning well child
visits, health referrals, and immunizations.
b) Assists Family
Development Specialists in working with parents
to achieve
compliance.

Coordinates services for children and families with behavioral
health concerns.
a) Provides
observations in the classroom, homes, or socializations to
determine behavioral health issues or needs.
b) Attends
program staffings to provide consultation on identified
children
with behavioral health needs as scheduled.

information received and observations conducted while
assessing and coordinating behavioral health services for
families
d)Arranges
screenings and assessments for children with suspected
disabilities and/or behavioral health needs.
e) Ensures that
all behavioral health information is filed in the child’s family
development file.
f) Maintains
strict confidentiality regarding all information pertaining to
behavioral/developmental health needs of the child and family.
g)
Coordinates with family development specialist and/or lead
teacher to arrange conferences with parents when informed of
concerns regarding children, family members, and staff as
needed.
h) Facilitates
the flow of information between Early Head Start/Head
Start and
parents after referrals have been made.
3)
Provides consultation for parents regarding
specialized assistance for children with atypical behavior or
development
a) Collaborates
with Service Providers to facilitate appropriate treatment to
children with disabilities or atypical behaviors.
b) Participates
in behavior planning for children with identified behavioral
health/disabilities needs, to work in conjunction with parents
and staff to develop a comprehensive plan of action to address
behavioral health issues.
c) Coordinates
with family development specialist and/or lead teacher to
arrange conferences with parents: 1) elicit parental input and
observation of their child’s behavior, 2) share staff
observations and 3) provide parent education on appropriate
responses to behavioral issues, creating a nurturing environment
and strengthening familial relationships.
d) Encourages
participation of parents when behavioral health interventions
are needed.
e) Facilitates
staff/parent communication regarding child behavior and
behavioral health needs.
4)
Supports a team approach by working with the Early
Head Start and Head Start Coordinators, Supervisors and staff to
meet Performance Standards and further the goals of the program
a) Consults with
staff on how to use findings from developmental, sensory, or
behavioral screenings to address identified behavioral health
needs
b) Attends all
appropriate meetings and workshops and serves on appropriate
committees relating to Early Head Start and Head Start upon
request
c) Identifies and
reports suspected cases of child abuse or neglect to Child
Protective Services, with copies to Head Start Director,
Supervisor of Health Services, Supervisor of Family Services and
Supervisor of Education.
d) Works with
Family Development Services staff to secure support services for
families with CPS reports are necessary
a) Keeps
supervisor, other coordinators, and Early Head Start staff
informed of all behavioral health activity
b) Responds to
Behavioral Health Alerts within 5 working days and follows up on
behavioral health needs according to the Behavioral Health
Process in the Policy and Procedures Manual
c) Helps to
monitor responses to recommendations for classroom interventions
and referrals for professional services.
5) Works
closely with health, disabilities and behavioral health
professionals in the community
a) Coordinates
with the health/behavioral health professional to whom
referrals are made to see that appropriate services/treatment is
received.
b) Collaborates
with Head Start/Early Head Start to develop health and
behavioral health services that meet Performance Standards
c) Develops and
maintains a resource record of behavioral health services in the
community including contact persons, services provided,
eligibility, referral procedures, and associated costs
6) Assists parents or family members in participation
of child behavioral health interventions when necessary
a) Attends
meetings with parent and behavioral health professionals as
needed
b) Makes
referrals for psychological assessments as needed
c) Conducts
parent training, education and referral as needed
d) Works with
Supervisors to locate sources of funding to pay for intervention
7) Works with service area coordinators to ensure
access to behavioral health services for parents
a) Implements
educational activities for staff and parents regarding
preventative behavioral health and focusing on
wellness rather than illness
b) Facilitates
access for parents to behavioral health consultation,
support
groups, and training opportunities in regards to
appropriate responses to behaviors, creating nurturing
environments, relationships in the home and understanding
typical
behavioral
health needs
c) Works with
the Supervisor of Education and Education
Coordinators to plan educational activities and offer resources
for
staff and
parents on behavioral health issues.
8) Works with other service
area coordinators to facilitate an integrated system of service
delivery to children and families
a) Attends Center Staffings/Child Development Team
meetings to discuss needs of individual children and families.
b) Documents a plan of action for children with health
needs and follows up to ensure that the plan is carried out.
c) Tracks requests for information from service Providers
e) Updates the Supervisor of Health Services concerning
follow up and referral.
e) Works with the Supervisor of Health Services to
provide technical assistance to staff concerning health issues
as requested.
f) Monitors all health related documentation and tracking
for compliance and service delivery.
g) Monitors computer data reports for accuracy.
9)Contacts Local Education Agencies (LEAs)
and community agencies to initiate and coordinate appropriate
screening, and referral for evaluations and services.
h)
Makes appropriate referrals to LEAs and community
agencies as needed.
i)
Transports or arranges for transportation of children and
families to appointments for evaluations
j)
Maintains agreement letters with LEAs
10) Plans
and delivers trainings on health and nutrition topics in the
classroom and at parent meetings as requested.
11 ) Monitors emergency and
health plans at each center for safety and compliance.
a) Inspects
classrooms, Health Information Notebooks, and kitchens for
posted information concerning allergies, emergency numbers, and
photo releases.
b) Meets with
center teams to plan for and monitor care plans for children
with special health needs.
c) Checks each
center’s first aid kits, restocking as needed bi-weekly.
d) Checks each
center’s medication log and records bi-weekly.

Makes home visits to families with specific health needs as
requested.
13) Monitors
staff health records at each center and at the Central Office.
a) Informs staff
and their supervisors of needed staff physicals, TB tests, and
First Aid/CPR re-certifications.
b) Updates staff
emergency contact sheets as needed.
